So, I finally got around to asking the daycare/kindy across the road if they'd like me to do some music lessons for them.

The lady I spoke to was keen, but admitted she's leaving in a week, so it would be best if I did up a "package" that they could send around their chain/leave for the next person.

I'm thinking cover letter & CV. Given that it's not a job I'm applying for, I don't know what format/length or anything. Maybe a brochure would be better?

I'm absolutely lousy at self promotion, so WDYT I should include on the cover letter/brochure?

I guess it kind of depends... at my daughter's old daycare centre, they had somebody come in and do music lessons that were opt-in, I think they were about $8 per week.

I think you should include more than a cover letter and CV. A brochure is a good idea, and looks more professional - you'll have to have something like that anyway, if you want to be able to sell it to parents. Give an outline of the "course" you'll be teaching, including how it relates to curriculum standards (not just ones relating to music but to other areas, if possible). Have a "theme" or idea for each term that you'll be exploring with kids (ie melody, rhythm, types of music, etc). I'd be expecting some decent structure and some proof that the teacher knew what they were doing with preschoolers.
