If his concession card is an FTB one, the children you both receive FTB for would be on the card but not other offspring that you don't get entitlements for.

His parents are the guardians and they earn too much or are have too many assets to have a health care card?

if his kids are on her card he can still take her card and buy the medication for them at the discounted price.

If he has the children every second weekend and half of all the school holidays, he will probably meet the criteria for a regular care child, which means he won't get a payment for them but he will get other entitlements, such as have them on his health care card.
But whoever is the FTB recipient needs to claim for them. They aren't just automatically added on because they are his kids.

Here is the info regarding a regular care child:

A regular care child is a child in your care for between 14 per cent and less than 35 per cent of the time. If you have a regular care child in your care, you will not be entitled to Family Tax Benefit Part A and Part B (including the supplements), Multiple Birth Allowance, Large Family Supplement or Baby Bonus.
You may be entitled to the following family assistance:
• Rent Assistance
• a Health Care Card
• the Lower Threshold of the Medicare Safety Net
• additional Remote Area Allowance with your Social Security payment (e.g. Newstart Allowance)
• Child Care Benefit
• Child Care Rebate.
To be eligible for these benefits you will need to meet the other requirements for that benefit and complete a claim.
