I'm trying to fill in passport application forms and I'm a bit confused by a couple of fields and am hoping someone can assist so I don't stuff it up.

On the first page I put my DOB and then it goes to a new page to fill in the details of the certificate.

Then I need to put in the date of registration of birth and the date of issue of certificate.

So I put my DOB in but not sure if the 'date of registration' is my DOB ie. 31/12/197X or the date mum informed the registry office ie. 07/01/197Y if so, what then is the 'date of issue'? Is that the date they gave mum the 'true copy of blah blah blah and signed it? ie. 17th April 197Z

DH's for example has DOB 20/03/197W, 'REGISTERING AUTHORITY: Name Registrar General Date: 19 April 197W and the certified copy was issued from BDM 18 May 1999.

Same deal with my marriage certificate - is the date the name change was registered the wedding date or the date under the REGISTERING AUTHORITY?

I suspect it's bloody obvious to everyone else - I'm just paranoid about getting it wrong!

Oh and does being a child on your mum's passport 30 years ago count as having previous Australian passport or travel document?

For the birth certificate yes I would say that is right

Your DOB, the date listed on your certificate as the registration date which would be usually within a month of you birthdate. The final date on the certificate is when the certificate was issued true copy. Just note that you need a full certificate and not a birth extract.

I don't think being on your mums passport is the same as having your own as 30 yrs ago you wold not have been able to travel without your mother. The passport was hers. But then again I may be wrong.

Your birth certificate should have a date of registration (which is when it was initially registered) then further down at the bottom it would have date of issue of certificate.

Don't worry howdo, I was confused too recently when faced with the same questions for a change of name form. I ended up putting the registration date and issue date as the same because I couldn't find 2 different dates on my child's birth certificate. You are not the only confused one. original.gif
